Advanced Care at Home (ACH) Program – UNC Health
About the Program
UNC Health’s Advanced Care at Home (ACH) program is redefining inpatient care by safely delivering hospital-level services in a patient’s home. Using a combination of virtual care, in-home clinical visits, and a centralized Command Center, ACH achieves clinical outcomes equal to or better than traditional hospitalization - while significantly improving the patient experience.
Patients receive access to many inpatient services, including medication management, mobile imaging, home infusion, lab services, rehabilitation, nutrition consults, and more -all without leaving home. Care is fully integrated with primary care providers, specialists, and ancillary services across UNC Health.
Your Role in the Program
As an Advanced Practice Provider in the ACH program, you will deliver high-quality inpatient care in a hybrid model that blends virtual medicine with in-home clinical care. You’ll collaborate closely with supervising physicians, Command Center nurses, and interdisciplinary partners to manage complex patients safely at home.
Incentives: This position qualifies for a $10,000 commitment incentive, paid in four (4) installments over two (2) year work commitment.
This position is also eligible for relocation assistance (based on location)

What You’ll Do
As an APP in this program, you will:
- Conduct initial virtual history & physical exams for new ACH admissions once patients arrive home
- Partner with supervising physicians to develop and implement individualized plans of care, including diagnostic testing and specialty consultations (virtual or in-person as needed)
- Perform in-home clinical assessments when clinically indicated
- Coordinate safe handoffs with the attending physician, Command Center RN, and the patient’s primary care provider
- Screen and evaluate eligible patients at brick-and-mortar facilities for ACH enrollment
- Educate patients and families on the ACH model, aligning care plans with patient values and goals
- Foster strong collaboration across physicians, nursing, and vendor partners
- Practice in accordance with all state, hospital, and credentialing guidelines
Program & Role Details
- The APP functions as part of an interdisciplinary healthcare team, practicing within privileges approved by the credentialing committee
- Care is delivered in collaboration with a supervising physician
- The role reflects UNC Health’s mission, vision, and values and complies with all regulatory, accreditation, and organizational standards
- Command Center – Parkline (Chapel Hill, NC) is considered the primary place of business
- Candidates must be local to Chapel Hill, NC, as this is a hybrid role requiring on-site and in-home presence

Education Requirements:
Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
Licensure/Certification Requirements:
Licensed as an Advanced Practice Provider (i.e., Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, Certified Registered Nurse Anesthetist) in the state of North Carolina.
Board certification if required by the Department. DEA.
Professional Experience Requirements:
No prior experience required.
Knowledge/Skills/and Abilities Requirements:
Demonstrated clinical competency.
Effective leadership skills. Strong interpersonal skills.
Ability to work independently and collaboratively.
Demonstrated written and verbal communication and strong analytical skills.
Effective interaction to ensure the successful care of patients in the clinical setting.
Documentation in the medical record to substantiate accurate billing.
