Overview

Providers work in a collaborative environment that integrates onsite care with telemedicine and remote patient monitoring to improve outcomes and reduce hospitalizations.
Responsibilities
- Provide comprehensive primary and geriatric care in residential care settings
- Manage chronic conditions, acute concerns, and routine visits
- Develop and implement individualized care plans
- Collaborate with nursing staff and interdisciplinary teams
- Participate in telehealth visits as needed
- Utilize remote monitoring tools for proactive patient care
- Complete accurate and timely EHR documentation
Requirements
- Active Nurse Practitioner or Physician Assistant license in Vermont
- Must have long term senior care experience
- Experience in geriatrics, long-term care, or post-acute care preferred
- Strong clinical and diagnostic skills
- Ability to work independently within a team-based model
- Comfortable with EHR systems and telehealth technology
- Strong communication and interpersonal skills
Compensation and Benefits
- $90 per hour
- No weekends, holidays, or call
