• Clinical Shift Schedule: Monday–Friday, 8:00 AM – 5:00 PM
PATIENT INFORMATION
• Patient Demographics: Primary Care Patients• Patient Volume/Census: Approximately 20 patients per day
FACILITY INFORMATION
- Support Staff Available: 4 Front Desk Staff, 3 Medical Assistants, and potential support from 2 APPs
- Physician Support: 1 Physician Practice
- Reason for Coverage: Maternity Leave Coverage
- Office Equipment Available: Standard outpatient clinic equipment
COMPENSATION & BENEFITS

JOB REQUIREMENTS
• Licenses: Active Connecticut license required
DUTIES & RESPONSIBILITIES
• Provide outpatient primary care coverage in a collaborative physician practice• Evaluate, diagnose, and treat patients in a primary care setting
• Manage approximately 20 patients per day
• Collaborate with physicians, APPs, medical assistants, and front office staff as needed
• Maintain accurate and timely patient documentation
• Adhere to all facility policies, compliance standards, and onboarding requirements
