Primary Care Nurse Practitioner or Physician Assistant

Start: ASAP
End: Ongoing
Schedule: Monday through Sunday, 7:00am to 7:00pm, with 5 to 8 assessments per day
Openings: 1
Travel and Lodging: Limited

Position Overview

• In home health and wellness assessments
• Adult, Family, and Geriatric Nurse Practitioners and Physician Assistants accepted
• Assessments typically last about 45 to 75 minutes
• Patient population is geriatric
• At least one weekday of availability required
• Weekend only availability is not accepted
• Travel required within assigned areas
• All documentation is completed on an iPad

Clinical Scope

• H and P evaluations
• Medication review
• Vital signs
• Finger stick testing
• Urine sample collection
• Fecal kit distribution

Required Qualifications

• Active state license
• National certification for NP or PA
• CAQH
• NPI
• Unrestricted license
• Auto insurance meeting client requirements
• No DEA required

Credentialing and Onboarding Items (must be provided)

• Credentialing is self paced and typically 30 to 45 days
• Online training modules must be completed before scheduling
• Required credentialing documents as requested by the client team
