Description
Holy Cross Medical Group (HCMG) and Holy Cross Hospital are seeking a skilled and compassionate Advanced Practice Provider to join our Oncology team. The ideal candidate will have experience or a strong interest in Survivorship care and will work collaboratively under the supervision of multiple Oncologists and other healthcare professionals to deliver high-quality, patient-centered care.
Key Responsibilities:
- Conduct comprehensive survivorship assessments including history, physical examinations, and interpretation of diagnostic studies.
- Develop, implement and regularly update individualized survivorship care plans following completion of cancer treatment.
- Summarize treatment history (including surgery, chemotherapy, radiation, and immunotherapy) and outline follow-up care
- Coordinate transition from active oncology care to long-term survivorship.
- Monitor for cancer recurrence and secondary malignancies
- Oder and interpret appropriate surveillance labs, imaging, and diagnostic tests.
- Identify and manage late and long-term effects of cancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.
- Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.
- Counsel survivors on health promotion, disease prevention, and age/risk appropriate screenings and vaccinations
- Provide patient and caregiver education.
- Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.
Qualifications:
- Graduate of an accredited Physician Assistant or Nurse Practitioner Program
- National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
- Current Physician Assistant or APRN license in the State of Florida.
- AOCNP Certification
- BLS/ACLS
Preferred Skills:
- 2 years of Oncology Experience
- Experience with Epic Electronic Medical Record
